To schedule a speech therapy evaluation, please follow the steps outlined below:

Contact our office or schedule an evaluation online:

Step 2
Provide necessary details:
Our receptionists will ask for relevant information to help us understand your specific requirements. This will ensure that we match you with the most suitable speech therapist for your evaluation.
Name (patient's name if different from caller)
Age
Preferred evaluation date and time
Any additional information or concerns you would like to discuss during the evaluation
Step 3
Intake & Insurance verification (if applicable):
If you have health insurance coverage, please have your insurance information ready when you call our office.
Our team will guide you through the insurance verification process, ensuring a smooth experience.
Speech Castle will collect the patient’s demographics (Contact Information, Patient’s Diagnosis/Condition and Insurance) and send over forms to be completed and signed.
Step 4
Verification:
Speech Castle verifies benefit coverage with the patient’s primary insurance.
If necessary, Speech Castle will help the family obtain a referral authorization from the primary care physician.
We will discuss the benefits quoted with the patient’s parent/guardian.
Step 5
Confirmation:
Once the scheduling process is complete, you will receive a confirmation email or text message containing the date, time, and location of your evaluation appointment.
Please review this information carefully and notify us immediately if any changes are required.
Step 6
Documentation:
On the day of your evaluation, please bring any relevant documentation that may help us understand your situation better. This can include medical records, previous evaluation reports, or any personal communication goals you may have.
We understand that life can sometimes get in the way, and if you need to reschedule your evaluation appointment, please notify our office at least 24 hours in advance to allow us to accommodate your request.
